Referrals
Complete the
WELCOMING REFERRALS FOR CLIENTS WHO ARE:
- Looking for an AT Assessment by an Occupational Therapist (OT)
- Impacted by a physical/neurological disability (e.g. cerebral palsy, spinal cord injury, spina bifida, motor neurone disease (MND), multiple sclerosis (MS), Rhett Syndrome, muscular dystrophy, Parkinsons, etc).
- NDIS self-managed or plan-managed; or self-funded
- Located within about 1-hour drive from Launceston, including George Town and Deloraine (services are provided in-home and travel costs apply)
- Any age
At this stage, WE can’t accept referrals for:
- Clients who are NDIS Agency-managed
- Functional Capacity Assessments, SDA/SIL Assessments, home modification assessments
- Assessments for AT needed to limit mobility (i.e. for restrictive practices), rather than to support someone with a physically-limiting disability.

About
Hi, I’m Amy Hilaire. When I was 10 years old, my grandfather had a severe stroke that left him with a physical disability. Having a front-row seat to his rehab and seeing how he has continued to live a full life despite his disability inspired me to become an Occupational Therapist.
I have focussed my career on finding creative ways to make life easier and more comfortable for people with complex disabilities and their families.
I love being an OT and helping people find the best wheelchairs, shower commodes, and other equipment for each person’s unique lifestyle.
